Report Cards and Supply Lists


Report Cards are mailed approximately within ten days following the last day of school. 

Also included with this mailing is a complete list of all the supplies required for each grade for the 2017/2018 school year.

Health Room - End of the Year Reminders



1.      Medications need to be picked up by the last day of school on Tuesday, June 6, 2017.

            Students entering seventh grade are required to have the following:


·      1 dose of Tetanus, Diphtheria, Acellular Pertussis (Tdap) - if five years has elapsed since the last Tetanus immunization.

·         1 dose of Meningococcal Conjugate Vaccine (MCV)

If your child(ren) have had a  physical or dental exam since September 2016 this exam qualifies.  Please have the pediatrician or dentist complete the form you received (or downloaded) and return it to the school.

Registration Information for New Students Entering Grades 1 to 8, Kindergarten, and Pre-School

Holy Name of Jesus School began accepting new student registrations for the 2017-2018 school year on Monday, January 30, 2017.

To register your child(ren) for Pre-School, kindergarten, and/or grades 1 through 8 call the school office 717-657-1704 to schedule an appointment.

Requirements for New Student Registrations:

  • New kindergarten students must be age 5 by August 31, 2017
  • Bring at the time of registration the following documents for all students entering kindergarten through grade 8:
    • Birth Certificate
    • Copy of Immunizations
    • Baptismal Certificate
    • Any necessary legal documents
  • New Pre-School students must be age 3 or 4 and potty trained by August 31, 2017
  • Bring at the time of registration the following documents for all students entering Pre-School:
    • Birth Certificate
    • Copy of Immunizations
    • Any necessary legal documents

Registrations cannot be accepted without the documents listed above.

Important Day Care Busing Information for 2017-2018

Families who reside in the Central Dauphin School District whose child(ren) ride the bus to/from Holy Name of Jesus School are required to complete a “Child Care Provider Request” form EVERY YEAR IF the child will be traveling to/from a daycare provider.   Day care providers must reside within the Central Dauphin School District boundaries.


Daycare accommodations that include busing do not carry over from one school year to the next; therefore, families who will require day care busing again for the 2017-2018 school year must submit a new form by the end of this school year. 

If daycare arrangements change during the school year, please fill out a NEW form.  To obtain this form contact the Holy Name of Jesus School office (717-657-1704) or use the link provided below.

2017-2018 Asbestos Notification

The Diocese of Harrisburg provides a Parent/Legal Guardian Notification to our school each school year that explains the Asbestos Hazard Emergency Response Act (AHERA) as it pertains to Holy Name of Jesus School.  Please read this document.

After School Program

ASPThe Holy Name of Jesus Schools’ After School Program (ASP) is a full time program.  Students receive a snack, have homework time, recess time, and participate in an activity.  The program hours are 3:30pm to 6:00pm.  Students must be picked up promptly.

The first day for the After School Program for the 2016-2017 school year: Monday August, 29, 2016

Previously Registered ASP Students

  • Any student/s who were registered in this program during the previous year may begin on the first day they have school.

New ASP Registrations

  • Any family who would like to register a new student/s may contact the school office (657-1704) for a handbook and registration forms or you may download the  After School Program Handbook.
  • Submit all registration forms to the school office.
  • Students may go to the After School Program on the same day they are registered.

Sick and Late Student Call-in Information

A parent/guardian is required to call the school by 9:00am to report sick or late children. If your child(ren) will not be attending school due to sickness or a late arrival to school due to an appointment or other reason, please use the following procedure:

  • Calls before 7:30am: when prompted press 3 for the Attendance Line.
  • Calls after 7:30am and before 9:00am ask for the “Attendance Line”, to report your child(ren) sick or late.

 Contacting the Teacher

Teachers may be contacted in the following manner:

1. Call the office (657-1704) during regular school hours.  The office will forward your call to voice-mail.  The teacher will return your call within 48 hours.

2. Send a note to the teacher via your child.

3. Send an e-mail to the teacher.


Receiving Correspondence from the School

During the school year, most correspondence will be sent home with the youngest child in grades one through eight.

For example:

If we have a school family with three children in grades six, five, and kindergarten – a flyer will be sent home with the fifth grade student. 

So, please check with your youngest child on a daily basis for any school related correspondence that may be coming home.

If you think you are not receiving correspondence:

1. Please contact the homeroom teacher of your youngest child.

2. Check our school website for the information that you need.


Car Rider Dismissal Procedures

On Monday, August 31, 2015 parents began parking outside the Primary Building for Car Rider Dismissal.  Parents line up outside the Primary Building in a single file line to enter at 3:00pm the left door to go into the former church.  Tables are set up at the top of the steps for parents to sign out their child. ID may be required. After the parent has signed out the child, the parent will go to another teacher on duty to call the child/children.  Parents and students will exit the former church through the middle door of the former church into the lobby to exit the building through the middle door.

If your child is a car rider each day, send a note to the homeroom teacher stating this information.  The homeroom teacher needs a note if your child is being picked up by someone other than a parent.  If your child is an occasional car rider, the homeroom teacher needs a note that morning.


Honors, and Effort Awards

1st HonorsTo achieve First Honors a student will have earned an A in all academic subject areas and a C or greater in all special subjects.

2nd Honor CardTo achieve Second Honors a student will have earned an A or B in any combination in all academic subjects and a C or greater in all special subjects.

Honorable MentionThe Honorable Mention achievement is subject to the discretion of the homeroom teacher and is awarded to those students who are doing their best.

To achieve overall honors at the end of the year, teachers also consider a student’s character development.


Obtaining Clearances for the Youth Protection Program

In order to ensure every child’s safety within the Diocese of Harrisburg, all persons who are volunteers at Diocesan schools and parishes must complete the following:

  • Diocesan Disclosure Statement
  • PA State Police Report
  • PA Child Abuse History Clearance
  • Mandated & Permissive Reporter Training
  • FBI Fingerprint Report (for those who have not resided in PA at any time within the past 10 years)

Forms and instructions to apply for clearances are available HERE, or can be picked up at the Parish Office. 
Once all clearance certificates are received they must be submitted to the Parish Office as a complete package. 
The office staff will submit the information to the Diocese of Harrisburg who will issue a Youth Protection Badge.  All “cleared” persons must wear the Youth Protection Badge when involved in any and all activities where children are present.
